Please review your biography paragraph, which was provided to CCAI by your selecting Member of Congress, to ensure accuracy of the information. If there is incorrect information please clearly mark your edit and send it via email or fax to Emily Collins at Emily@ccainstitute.org or 202-544-8501. Please note: if you have not submitted any corrections by September 1 st, your biography will be considered approved by you and will be used for print and media distribution.

Media Release Form
Please sign and return the Media Release Form which provides your permission for CCAI to use your information in conjunction with the Angels in Adoption™ program. The form must be submitted by September 1 st in order for your biography to be included in the program book or for your story to be included in local and national press releases of the Angels in Adoption™ Gala. You may email or fax the form to Emily Collins at Emily@ccainstitute.org or 202.544-8501.

Pictures
For the 2007 Angels in Adoption™ Gala presentation, we request a picture of you, your family, and/or organization. It is advised that you submit two or three pictures so that the most appropriate and highest quality photo may be chosen for use. Please submit your pictures no later than September 1 st. For your convenience, pictures may be sent electronically to Emily@ccainstitute.org or through the mail to Emily Collins, 311 Massachusetts Avenue NE, Washington, DC, 20002.
